What is a Community Health Center?

·        Community Health Centers (CHCs) are low-cost medical clinics located in underserved areas. They are open to anyone regardless of insurance status. Whether you have health insurance or not, everyone is welcome to use community health centers. There are over 1,500 CHCs nationwide.

·         If you are uninsured or underinsured, you may be eligible to pay for services based on your income level and family size on a sliding scale basis.

·         CHCs offer low-cost medical, dental and behavioral health care.

·         CHCs receive most of their funding from the federal Health Resources and Services Administration’s (HRSA) Health Center Program.

·         Many CHCs have adopted the patient-centered medical home (PCMH) model to improve the health of medically underserved communities and reduce health care disparities, by delivering high-quality, cost-effective primary care. DBFPC is accredited as a PCMH through the Accreditation Association for Ambulatory Health Care.
